*A response of “yes” to any of the questions: “In the last 12 months, did you purchase cigarettes over the Internet for your own use?”; “In the past 12 months, have you or a friend or relative regularly purchased cigarettes for your own use on an Indian reservation?”; “In the past 12 months, have you or a friend or relative regularly purchased cigarettes for your own use in another state?”; “In the past 12 months, have you or a friend or relative regularly purchased cigarettes for your own use in another country?”.
