The sample was designed to recruit adults from three ethnic groups in two age categories, approximately equal numbers of respondents in two groups, <30 and >30 years of age. During analysis, the older age group has been further subdivided into two age categories, middle aged (31–59 years) and older people >60 years. The number of replacements during phase 2 is designated by R for each age/ethnic category.
